Özet
A 49-year-old patient presented with effort-related chest pain and dyspnea. Selective coronary angiography was performed. During the procedure, an accessory coronary vessel originating from the ostial region of the left main coronary artery compatible with a high origin diagonal was visualized (Picture 1). For further anatomical analyisis with the surrounding structures, multiple detector-row computed tomography angiography on a 16-channel scanner was performed. Volume-rendering images showed a diagonal coronary artery originating from the left main coronary artery. The left anterior descending and the left circumflex arteries were in the typical location (Picture 2a, 2b). To the best of our knowledge, this is the first case of a diagonal artery originating from the left main stem although septal branches originating from the left main artery have been reported. Multidetector row-computed tomography is an important method for further investigation of the relationship between coronary arteries, providing more information on their origin and course.
