| Tremor means synchronous contractions of the reciprocal muscles of a body part which are the resulting rhythmic oscillations. Levodopa is a prodrug that is converted to dopamine by DOPA decarboxylase and is used to treat ParkinsonLs disease. Tremor is a rare adverse effect of levodopa. In this report, a case with bilateral tremor in hands which is worsened after levodopa treatment is described.Case presentation: A 79-year-old male patient was admitted with complaints of bilateral tremor in hands and forgetfulness to an outpatient geriatric clinic. He had a history of tremor for a long time, but it got worse in the last two weeks. Rasagiline (1× 1 mg) and levodopa (3× 125 mg) were initiated to the patient for treating ParkinsonLs disease three months and one month ago, subsequently. Out of these drugs, he was on metformin (2× 1 gr), Ramipril 5 mg+ hydrochlorothiazide 12.5 mg (1× 1), and acetylsalicylic acid (1× 100mg). Family history revealed that his father and brother had tremor on bilateral hands. On examination, he looked anxious and had bilateral hand tremor, which appeared during movement. Mental status assessment showed that he had cognitive deficits in domains, including memory, orientation, perception, and attention. His scores on the Mini Mental status Examination and the Generalized Anxiety Disorder scale were 23 (range 0-30) and 16 (range 0-27), subsequently. Laboratory data did not show any abnormalities.
