In patients with chronic renal disorders who are in need of hemodialysis, vascular access is one of the primary problems confronted by physicians. In the formation of arteriovenous (AV) fistulas - the preferred choice for chronic hemodialysis - the preferred site should be the most distal location available, the "anatomic snuffbox." We, however, have performed a direct AV fistula construction at an even more distal location. which we have named the "distal snuffbox." In this study, 96 patients with chronic renal insufficiency received distal snuffbox AV fistulas. Thirty-two patients whose fistulas were insufficient underwent a second operation on the same extremity using the conventional snuffbox or Brescia-Cimino methods. A polytetrafluoroethylene (PTFE) loop arteriovenous graft (AVG) fistula was constructed in two patients who did not benefit from the other methods. The one-year patency rate for the distal snuffbox AV fistulas was 66.6%, a rate comparable with the other studies in the literature. Venous hypertension and the steal syndrome were minimal. This new method has several advantages, such as maximal arterialized vein length, and provides an additional opportunity for the probable secondary intervention. We believe that the success rates of this special method will ascend steadily as the case numbers and our experience increase. |
