Combined Brucella melitensis and Staphylococcus aureus Endocarditis in A Patient with A Prosthetic Aortic Valve with Fatal Outcome

Özet
A 58-year-old man with fever and fatigue for 15 days and who had a prosthetic aortic valve was hospitalized in order to detect the origin of the fever. After three blood cultures, ampicillin-sulbactam therapy was started. Brucella slide agglutination tests were negative in this period. Echocardiography demonstrated vegetation on the mitral valve. In all blood cultures, methicilin-sensitive S. aureus was isolated and infective endocarditis was diagnosed. The fever decreased on day 3 of anti-staphyloccocal cephazolin therapy, but on the 5th day of hospitalization, it increased to 40-C. Vancomycin therapy for meticillin-resistant S. aureus or any other agent associated with the prosthetic valve was initiated. On the 2nd day of vancomycin therapy, the patient died due to acute myocardial infarction leading to acute pulmonary edema.
